Key Responsibilities
The Registered Nurse will:
- Deliver nursing and Assertive Community Treatment (ACT) case management services within the community.
- Evaluate client needs, develop, and oversee Person-Centered Plans.
- Maintain accurate and timely documentation of treatment in an electronic medical record system.
- Assist in planning, linking, advocating, coordinating, and monitoring services to help clients access health care, financial aid, employment, education, social services, and natural supports.
- Provide after-hours video or audio assessments on a rotating schedule among team members.
Qualifications Required
The ideal candidate will possess:
- An Associate's Degree from an accredited nursing program and licensure as a Registered Nurse by the Michigan Department of Commerce.
- A minimum of one year of experience as a Registered Nurse.
- Proficient computer skills along with strong written and verbal communication abilities.
- Excellent organizational skills and the ability to work both independently and collaboratively within a team.
- Reliable transportation for community outings and client transport.
- Preferred experience working with individuals facing mental health challenges and/or substance use disorders.
