Eligibility and Intake Coordinator

UMOS is seeking a highly skilled Intake and Eligibility Specialist to join our team. As a key member of our organization, you will be responsible for determining the eligibility of potential beneficiaries and ensuring that they meet the program requirements.
Key Responsibilities:- Recruit, enroll, and determine the eligibility of potential beneficiaries.
- Conduct beneficiary orientations and coordinate the collection of necessary documents.
- Schedule initial intake sessions with beneficiaries and their families to determine eligibility.
- Review pre-applications and assist participants in collecting documents to verify their eligibility.
- Request documentation to determine client eligibility and forward requests for payment approval.
- Participate in case reviews and perform follow-up with beneficiaries as needed.
- Build relationships with participants and refer them to other programs as necessary.
- Implement and monitor ongoing recruitment activities to ensure beneficiary enrollments are achieved.
- Interpret policy to ensure compliance with federal laws and regulations.
- Coordinate and implement the enrollment process of beneficiaries, ensuring that eligibility is 100% accurate.
- Maintain accurate record-keeping systems and ensure all eligibility documentation is uploaded prior to submitting for payment processing.
- Implement and monitor an effective electronic filing system to safeguard confidential information.
- Ensure timely data entry of services and submit complete and accurate reports.
- Associate degree or two years of post-secondary education in social science, human services, or a related field.
- At least three years of experience in determining eligibility and/or case management.
- Bilingual in Spanish and English (oral and written).
- Proficient computer skills, including Microsoft Office Suite/Office 365.
- Good oral and written communication skills.
- Reliable transportation, a valid driver's license, and adequate insurance.
- Ability to work irregular and/or flexible hours.
