About the RoleThe successful candidate will be responsible for:
- Coordinating clinical and pharmacy activities for patients
- Assessing patient conditions and providing or assisting with patient evaluation and treatment
- Providing patient education and conducting in-home visits
- Collaborating with our clinical team to ensure seamless care delivery
