Key Responsibilities- Evaluate and assess the nursing needs of each client, developing individualized care plans to ensure optimal health and well-being.
- Administer medications, treatments, and other nursing interventions as prescribed by physicians and in accordance with established policies and procedures.
- Monitor and record vital signs, medical history, and other relevant information to ensure accurate and comprehensive care.
- Collaborate with interdisciplinary teams to coordinate care and services, ensuring seamless transitions and optimal outcomes.
- Provide education and support to clients and their families on health promotion, disease prevention, and self-care techniques.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance nursing practice and patient care.
- Current licensure as a Licensed Practical Nurse in the State of New York.
- Experience working in community residence or nursing home settings preferred.
- CPR certification required.
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations and teams.
- Ability to maintain confidentiality and handle sensitive information with discretion.
The Salvation Army offers a competitive salary range of $30.00 - $37.00 per hour, depending on experience. We also offer a comprehensive benefits package, including medical, dental, and vision coverage, as well as opportunities for professional growth and development.
