Job Summary:

We are seeking a qualified Licensed Practical Nurse (LPN) to join our team at Outreach Development Corporation. The successful candidate will work in our adolescent residential facility, providing high-quality patient care and contributing to a safe and healthy environment.

Key Responsibilities:

  • Conduct and/or monitor medication calls and health calls as scheduled.
  • Work with clients and guardians to schedule medical appointments and conduct follow-up as required.
  • Collaborate with the agency psychiatrist regarding client medication and pharmacy refills.
  • Provide on-site health services to address emerging and/or current health needs of clients.
  • Train and guide key staff members in emergency health situations.
  • Establish and monitor health-related policies and procedures.
  • Provide immediate first aid in health emergencies and escort clients to emergency facilities when necessary.
  • Conduct health assessments and make recommendations on client participation in treatment.

Requirements:

  • NYS Licensed Practical Nurse (LPN) certification.
  • Experience working in a substance abuse treatment setting or special populations.
  • Ability to work with high-risk adolescents and occasional holidays.
  • Flexibility in scheduling and ability to work effectively in collaboration with diverse groups.
