Geriatrician - Outpatient Clinic-Based Consults

About the Role:
Hartford HealthCare (HHC) is seeking a skilled Geriatrician to join our multi-disciplinary team serving patients in Windham, CT. As a Geriatrician with HHC, you will provide 100% team-based outpatient geriatric consults and SNF consults.
Key Responsibilities:
- Provide high-quality, patient-centered care to geriatric patients in outpatient clinics and SNF settings.
- Collaborate with a team of healthcare professionals to develop and implement comprehensive care plans.
- Participate in educational programs and professional development opportunities to stay up-to-date on the latest geriatric medicine practices.
- Contribute to the development of policies and procedures to improve patient care and outcomes.
What We Offer:
- A competitive salary range of $240,000 - $345,000.
