Nurse Specialist for Intellectual and Developmental Disabilities

Job Summary:

The Community Based Nurse will work with individuals diagnosed with intellectual/ developmental disabilities (I/DD) to provide health care supports, training, and technical assistance.

This role requires a registered nurse with a valid Pennsylvania nursing license and at least two years of experience working with individuals with I/DD.

The ideal candidate will have experience in providing training in health care issues and be able to work in partnership with agency nurses, health care coordinators, and supports coordinators.

Key Responsibilities:

  • Provide clinical consultation for individuals with complex health conditions
  • Act as a resource regarding medical/dental treatment, care, and follow-up
  • Complete nursing reviews and obtain certification in developmental disabilities nursing certification (CDDN)
  • Establish alliances with other nurses and healthcare coordinators
  • Participate in nurse network and other health-related meetings

Training and Technical Assistance:

The Community Based Nurse will provide training in physical and behavioral health care issues for individuals diagnosed with I/DD, staff members, family members, and supportive personnel.

They will also provide technical assistance around PCHC initiatives such as Behavioral Health Team Review of Psychotropic Medication form and Health Promotion Activity Plans (HPAPs).
