Job Summary

AlignMed Partners is seeking a highly skilled Medical Director to lead our clinical team in a sub-acute, long-term care setting. As a key member of our leadership team, you will be responsible for promoting high-quality patient care, improving clinical outcomes, and driving operational efficiency.

Key Responsibilities
  • Provide clinical leadership and oversight to ensure high-quality patient care and outcomes
  • Collaborate with regional medical directors and center administrators to develop and implement clinical programs and initiatives
  • Develop and maintain strong relationships with healthcare providers, payers, and other stakeholders to ensure seamless care transitions and coordination
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety
  • Stay up-to-date with industry trends, best practices, and regulatory requirements to ensure compliance and excellence in clinical care
Requirements
  • Board certification in Internal Medicine or Family Medicine, with maintenance of certification
  • Geriatrics specialization or equivalent experience in long-term care
  • Certified Medical Director (CMD) through AMDA or equivalent experience
  • Strong leadership and communication skills, with the ability to work effectively with diverse stakeholders
  • Proven track record of improving clinical outcomes, reducing readmissions, and enhancing patient satisfaction
