We are seeking a compassionate and skilled Home Health Physical Therapy Professional to join our team at AccentCare. This role involves providing essential home health care by implementing care plans and educating patients and their families on safety and medical equipment use.

About the Opportunity:

This position offers a competitive salary of $61,500 - $69,000 per year, paid on a per-visit basis. As a Home Health Physical Therapy Assistant, you will have the opportunity to work in Adams and Jefferson counties, making a meaningful impact on the lives of our patients and their families.

Job Description:

As a Home Health Physical Therapy Assistant, your responsibilities will include:

  • Implementing care plans and communicating any findings or changes in regards to patients back to the Physical Therapist and physician
  • Educating patients and their families in basic home safety and medical equipment safety

Qualifications for this role include:

  • An Associates degree from an accredited school of Physical Therapy
  • A valid license, registration, or permit to practice in the state of agency operation
  • Valid CPR certification while employed in a clinical role
  • The ability to travel to various business locations
