Your Role

As a Home Health Physical Therapist Assistant, you will play a critical role in implementing care plans and educating patients and families on safety and wellness. You will work closely with our Physical Therapists to ensure the highest quality of care and support for our patients. Key Responsibilities

* Implement care plans and communicate findings or changes to the Physical Therapist and physician
* Educate patients and their families on basic home safety and medical equipment safety
* Work collaboratively with the Physical Therapist to ensure the highest quality of care and support for patients
* Maintain accurate and timely documentation of patient progress and care plans

Requirements

* Associates degree from an accredited school of Physical Therapy
* Valid license, registration, or permit to practice in state of agency operation
* Current CPR certification
* Ability to travel to all business locations
