Key Responsibilities- Conduct thorough assessments of individuals' support systems and service needs, including participation in interdisciplinary team meetings as needed.
- Develop and implement person-centered plans to ensure ongoing care, treatment, and support.
- Collaborate with individuals, legally authorized representatives, and other stakeholders to ensure effective communication and information exchange.
- Provide training and education to promote the exercise of rights and independence.
- Assist with the coordination of admissions, transfers, transitions, and discharges as needed.
- Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy.
- Ability to work under moderate supervision with limited latitude for the use of initiative and independent judgment.
