Adecco Healthcare is currently seeking an Advanced Practice Nurse. Below are the details of the position:


Compensation

$108,000 - $135,000 annually


Work Schedule

Home Visit Physician Department

  • Must be capable of conducting patient visits in their residences
  • Monday to Friday, 8:30 AM to 4:30 PM

Role Summary

In this Advanced Practice Nurse position, you will:

  • Provide expert consultations to patients.
  • Conduct comprehensive follow-up appointments.
  • Perform in-person patient assessments across diverse healthcare environments.
  • Adapt to various settings while ensuring high-quality, personalized patient care.

Key Responsibilities
  • Conduct follow-up visits in patients' homes and care facilities.
  • Perform health assessments appropriate for each visit.
  • Diagnose and manage patient symptoms effectively.
  • Issue referrals and orders for laboratory work, diagnostic tests, durable medical equipment, home care services, physical therapy, and occupational therapy.
  • Facilitate referrals to additional services as necessary.
  • Maintain effective communication with specialists involved in patient care.
  • Complete electronic medical records promptly, in accordance with company policies.
  • Represent the company positively to patients, families, and the community.
  • Engage in performance improvement and quality assurance initiatives.
  • Attend mandatory staff meetings and training sessions.
  • Participate in rotating on-call coverage.
  • Perform other assigned duties as required.

Qualifications
  • New Jersey license or multistate license.
  • DEA and CDS licenses required upon hire (assistance with licensing fees provided).
  • Ability to commute (reimbursement available).
  • Experience in palliative care, oncology, hospice, or geriatric care preferred.
  • Master's Degree in Nursing.
  • Proficiency in electronic medical records.
  • Bilingual skills are a plus.
