About the Job: As a Community Care Manager, you will play a vital role in ensuring that patients receive the best possible care. You will visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices, to assess and coordinate their care needs.

Responsibilities:

  • Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team.

Requirements:

  • RN in the state of Texas, Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Masters Social Worker (LMSW).
  • 2+ years of experience working within the behavioral health setting.
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
  • Reliable transportation and the ability to travel up to 75% of the time in this assigned region.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
