About the Role:

  • We are seeking a skilled Physical Therapist to join our team at Lake Pointe Health Center.
  • As a Physical Therapist, you will be responsible for evaluating and treating patients with physical disabilities or impairments.

Responsibilities:

  • Evaluate patient medical charts and develop treatment plans in accordance with their needs.
  • Implement treatment plans through direct treatment and education of the plan of care to all team members.
  • Attend interdisciplinary meetings to relay clinical findings and make recommendations to other members of the rehab team.
  • Conduct staff and family training and education when appropriate.

Requirements:

  • A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program.
  • Hold a valid or pending State Physical Therapy License.
