Position Summary:
The role of a Comprehensive Care Physician at Oak Street Health is to deliver equitable and effective healthcare solutions to local Medicare patient populations within our innovative network of community-based primary care facilities. Our physicians work within a single medical center supported by extensive care teams, allowing them to concentrate on providing superior quality care rather than merely increasing service volume. Our model of value-based care and competitive incentive structures are designed to reward positive health outcomes, minimize hospital admissions, promote preventive care, and ensure an exceptional patient experience.
We seek physicians who are deeply motivated to deliver outstanding care to older adults. Recent graduates are welcome to apply.
Highly qualified candidates may also be considered for a Center Medical Director role.
Key Responsibilities:
- Evaluate and diagnose patients at our local clinics
- Oversee and manage primary care services
- Prescribe and administer medications and treatments
- Maintain Patient Electronic Medical Record data
- Collaborate with regional and central leadership to achieve health quality objectives
- Work alongside Practice Managers to lead and manage the center care team
- Perform additional duties as assigned
Required Qualifications:
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
- Board Certification in Internal Medicine or Family Medicine (or board eligible)
- Active, unrestricted State License
- Active DEA license
- Authorization to work in the U.S.

Preferred Qualifications:
- Fellowship training in Geriatrics
- Experience working with Geriatric populations
- Familiarity with Value-Based Healthcare Models
- Experience in collaborative environments to ensure positive health outcomes
- Experience in outpatient primary care settings
- Bilingual proficiency where applicable
