Clinical Care Coordinator, Pediatric Services

Boston Childrens Hospital is seeking a skilled Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will be responsible for coordinating patient care and facilitating effective transitions between healthcare settings.
About the Role
This is a per diem position requiring collaboration with inpatient and ambulatory teams to optimize delivery of services. The ideal candidate will possess strong clinical knowledge and skills, as well as excellent communication and interpersonal abilities.
Responsibilities
- Assess and plan daily clinical operations to address comprehensive health needs of patients and families
- Collaborate with clinic teams and inpatient case managers to ensure seamless care coordination
- Integrate regulatory requirements, length of stay, resource allocation, funding sources, and capacity management to optimize service delivery
- Participate in departmental and multidisciplinary programs, committees, and special projects
Requirements
- Bachelor's degree in Nursing (BSN); Master's in related field preferred
- Minimum 5 years nursing experience; pediatric experience preferred
- Certification as a Certified Case Manager preferred; obtain within first two years of employment if not certified
- Experience as a Case Manager (Discharge Planning and/or Utilization Management) preferred
Salary and Benefits
The estimated salary for this role is $85,000 - $110,000 per year, based on location and experience. Additional benefits include opportunities for professional growth and development, competitive compensation packages, and a collaborative work environment.
