Key Responsibilities- Administer prescribed medications, treatments, and therapies to patients as directed by physicians and other healthcare professionals.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Educate patients and their families on disease management, self-care techniques, and prevention strategies to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Active LVN license in good standing.
- At least six months of recent experience as a Nurse in a clinical care setting.
- Home health experience preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
