Job DescriptionThe Resource Nurse will assist the Clinical Manager in daily triage activities, including referrals, admissions, and patient care monitoring. This role involves participating in interdisciplinary meetings, reviewing meeting notes, and assisting with medical record audits to ensure quality care compliance.
This position requires strong communication skills for receiving inquiries, performing telephone assessments, and developing plans to address caller needs. The Resource Nurse will also serve as a liaison between Hospice and other professionals, enhancing team integrity and clinical practice.
Key Responsibilities:- Receives and triages inquiries from referral centers, staff, patients, caregivers, and referral sources.
- Develops and implements plans to address caller needs, including instruction, dispatching staff, or contacting physicians.
- Assists the Clinical Manager and Regional Executive Director in liaising between Hospice and other professionals.
- Enhances team integrity by providing a consistent role model and resource person.
- Evaluates own professional growth and development needs to maintain competence.
- Participates in quality improvement activities and ongoing education to develop new skills.
Experience Requirements:
Three years of Hospice/Home Health experience is required, with a minimum of two years of related management experience and four years of medical surgical and/or oncology experience preferred.
Education Requirements:
An LPN education from an accredited nursing program is required, with a BSN RN preferred.
Required Certificates and/or Licenses:
Licensure as a nurse with licensure from the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia is required.
