Job Title: Pediatric Community Health Worker

Job Summary:

Central Oregon Pediatric Associates (COPA) is seeking a highly motivated and compassionate Pediatric Community Health Worker to join our team. As a key member of our Patient-Centered Primary Care Home, you will play a vital role in connecting our pediatric patients and families to community social services, improving health equity and outcomes.

Key Responsibilities:

  • Maintain an in-depth knowledge of principles, practices, standards, and techniques in accordance with COPA policies and procedures.
  • Screen and assess health status and social determinants of health during appointments and referrals.
  • Assist patients with completion of forms and paperwork.
  • Document SDOH screen results in electronic tracking systems.
  • Provide patient and family navigation to healthcare, social service systems, and community resources.
  • Knowledge of local population demographics, assets, and needs.
  • Knowledge of local health disparities and resources to address those disparities.
  • Place referrals to community resources, following up with patients and families.
  • Assist patients and families with scheduling appointments and provide advocacy to address health disparities.
  • Management of patient information in computer systems.
  • Create connections between vulnerable populations and healthcare providers.
  • Provide culturally appropriate health education on chronic disease prevention, physical activity, and nutrition.
  • Advocate for underserved individuals or communities to receive services and resources.
  • Collect data and relay information to department leadership to inform programs, procedures, and policies.
  • Effectively communicate in writing and verbally with patients and co-workers.
  • Attend team meetings and communicate with county and community-based agencies.

Requirements:

  • High School diploma or GED required; college level coursework in public health or healthcare related field preferred.
  • OHA Community Health Worker certification required.
  • Pediatric CHW experience preferred.
  • Exceptional communication skills and ability to interact with diverse populations.
  • Attention to detail and strong time management skills.
  • Ability to multi-task and work independently.
  • Strong analytical, problem-solving, and decision-making skills.
  • Knowledge of Patient Centered Primary Care Home and Care Coordination curriculum.
  • Clinical informatics knowledge and skills.
  • Epic EMR experience preferred.
  • Proficient with Microsoft Office Suite.
  • Bilingual in Spanish and English a plus.

Working Conditions:

Standard medical office; may be eligible for combination of on-site and home office. Travel between COPA clinics and business office as required.
