Job Summary

The Case Manager provides comprehensive community-based case management services, including trauma-informed case management for individuals returning to the community from incarceration. This role serves as a liaison to the District of Columbia Department of Corrections and Anchor Mental Health to identify and coordinate community resources and make referrals.

Key Responsibilities
  • Interview and assess the needs of returning citizens and individuals referred by jails or prisons, primarily in Wards 7 & 8, who have mental health needs and/or a history of addictive behaviors.
  • Meet with community partners to identify areas for collaboration and develop plans for coordinating services.
  • Ensure participants engage in group sessions to address personal aspects of their lives, such as parenting, identity development, relationships, family dynamics, and conflict resolution.
  • Foster collaborative relationships with organizations and service providers to share resources that support men and women transitioning to the community.
  • Collect and track data elements from each participant.
Requirements
  • Bachelor's degree in a mental health discipline.
  • 3 years' experience working with incarcerated adults, including those with mental disorders.
  • Valid driver's license and clean driving record.
