Job Title: Nurse Practitioner

Job Summary:

Monogram Health is seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a Nurse Practitioner, you will play a critical role in delivering high-quality, patient-centered care to individuals with chronic kidney disease and related metabolic disorders.

Responsibilities:

  • Conduct comprehensive annual wellness exams and provide personalized care to patients in their homes and virtually.
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives.
  • Document patient information, including chief complaint, diagnosis, past medical history, and plan of care.
  • Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians to deliver evidence-based care.
  • Participate in integrated care team meetings to discuss patient care and share best practices.

Requirements:

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Ability to work autonomously and practice without direct supervision.
  • Access to transportation, a valid driver's license, and car insurance.
  • Proficiency with medical instruments and equipment required by the work.
  • Knowledge of computer-based data management programs and information systems.
