The Nurse Practitioner is a vital member of our healthcare team, responsible for delivering high-quality patient care and collaborating with other healthcare professionals to ensure comprehensive treatment plans. This role requires a strong foundation in nursing practice, with a focus on diagnosing and managing patient conditions, developing treatment plans, and providing education and support to patients and their families.
Key Responsibilities- Conduct thorough health histories and physical exams to identify patient needs and develop treatment plans
- Diagnose and manage patient conditions, including ordering and interpreting diagnostic tests and prescribing medications as needed
- Develop and implement treatment plans that address patient needs and promote optimal health outcomes
- Provide education and support to patients and their families, including counseling and health promotion
- Collaborate with other healthcare professionals to ensure comprehensive patient care and coordinate referrals as needed
- Master's degree in Nursing or related field
- Current licensure as a Registered Nurse in the State of Michigan
- Specialty certification as a Nurse Practitioner
- Previous experience in a FQHC setting preferred
- Excellent communication and interpersonal skills
