Job Summary

This is a healthcare administration role, primarily responsible for managing the day-to-day operations of a team of chronic care management nurses. Key Responsibilities
  • Team Management: Manage and mentor the clinical supervisor team to ensure seamless day-to-day operations.
  • Clinical Resource Provision: Provide the necessary clinical resources to support the team in delivering optimal patient care.
  • Patient Liaison: Serve as a clinical liaison for patients, engaging with them on behalf of the care coordination team as needed.
  • Productivity and Performance: Work with the GM of the business unit and clinical supervisors to meet productivity goals, and, where needed, hold team members accountable for performance targets.
  • Daily Stand-ups: Conduct daily stand-ups with Clinical Supervisors to monitor progress and address challenges.
  • Departmental Collaboration: Active participation in company and department meetings, providing insights and updates from the clinical department.
  • Process Development: Collaboration with colleagues to develop and refine departmental processes and policies.
  • Productivity Tools: Enable the clinical team with productivity and time management tools to ensure success.
Supervisory Responsibility

This position has supervisory responsibilities over a group of remote Care Coordinator Supervisors.

Work Environment

This job operates in a professional environment. This role routinely uses standard office equipment.

Physical Demands

This is largely a sedentary role; however, some equipment setup may be required.

Position Type and Expected Hours of Work

This is a regular, full-time position.

Travel

This job is remote but may require occasional travel to attend company meetings.

Required Education and Experience
  • At least 3 years of managerial experience.
  • Hands-on experience with Electronic Medical Records.
  • Knowledge of Microsoft Office 365 including Outlook, Excel, and SharePoint.
  • Understanding and comfortability working with Phrase Express and WOS.
  • Experience using various technology platforms.
  • Able to exercise initiative, judgment, organization, problem-solving, and decision-making skills are necessary to succeed in this role.
  • BSN strongly preferred.
  • Excellent verbal, written, and listening skills are a must.
