VNS Health is seeking a skilled LPN/LVN to provide high-quality care to patients in their homes. As a per diem LPN/LVN, you will have the opportunity to work with a diverse range of patients and make a meaningful difference in their lives.
About the Role:- Provide one-on-one care to patients in their homes, working closely with our registered nurses to develop and implement individualized care plans.
- Monitor patient progress, report health changes, and adjust care plans as needed.
- Deliver personalized nursing and care management, evaluating patient needs and adjusting plans accordingly.
- Practice in the community, traveling to patients' homes or care facilities, using effective time management and agility to manage weekly caseloads.
- Use technology, including a tablet computer, phone, and other systems, to coordinate and maintain accurate and up-to-date electronic medical records (EMRs) for patients in the community.
- License and current registration to practice as an LPN in New York State.
- High School diploma and completion of LPN program.
- Minimum of one year of work experience as an LPN.
- Demonstrated ability to solve problems and think critically to support patient outcomes.
- Experience with electronic charting in HHAeXchange, preferred.
