About the Role:

  • Collaborate with company physicians, patients' other physicians, and providers to develop complex plans of care.
  • Provide clinical and non-clinical support to patients in palliative care within a designated territory.
  • Develop and implement treatment plans based on patients' goals and current conditions.
  • Meet patients' and families' physical and psychosocial needs with support from our inter-disciplinary team.
  • Coordinate optimal care and resources for patients and their families in a timely manner.

Requirements:

  • Current, unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in the state of Indiana.
  • DEA registration or eligibility required.
  • CPR certification required.

Preferred Skills and Experiences:

  • 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care.
  • Experience working with Electronic Medical Records (EMR) preferred.

Work Environment:

This is a field-based position, requiring visits to patients/members in Terre Haute and Bloomington, Indiana. Eligible for mileage reimbursement.
