Job Summary

AdvisaCare is seeking a compassionate and dedicated Home Health Care Specialist to provide assistance in daily living and personal care services in patient homes in accordance with an established care plan.

Responsibilities
  • Provide health care services in patients' residences, including assistance with daily living activities and personal care.
  • Perform domestic and household tasks, such as cleaning and laundry, to maintain a safe and healthy environment.
  • Transport and accompany patients to medical appointments and hospital visits.
  • Administer simple prescribed medications and assist with clients' personal care activities.
  • Monitor patients' vital signs, temperature, respiration, and other health indicators, and report on their condition to the care team.
  • Maintain accurate and detailed records of provided services and patient care.
  • Assist patients with mobility and physical therapies, exercises, and other activities to promote independence and well-being.
  • Instruct and counsel patients and families on healthy diet and exercise habits.
  • Provide companionship and basic emotional or psychological support to patients and their families.
Requirements
  • High School Graduate or equivalent.
  • Reliable transportation and a valid driver's license.
  • Ability to work independently and as part of a team.
  • Good communication and interpersonal skills.
  • Ability to travel within the service area.
  • CPR certification.
  • Ability to pass a drug screen and background check.
  • Bowel Program Experience is a plus.
  • Female Only caregivers.
