Job Summary

Custom Home Health Inc is seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will provide personalized care to patients in their homes, assisting with daily living activities and promoting independence.

Key Responsibilities
  • Provide hands-on personal care, including bathing, back rubs, oral hygiene, and skin care
  • Assist patients with dressing, undressing, and toileting activities
  • Prepare and serve nutritious meals, and assist with feeding if necessary
  • Follow personalized care plans, collaborating with nurses and the interdisciplinary team
  • Report on patient condition and significant changes to assigned nurses
Requirements
  • Valid certification through the state
  • 1+ years of certified nursing experience
  • Ability to read and follow written instructions and document care given
  • CPR certification
  • Demonstrates tact, patience, and good personal hygiene
