Job Summary

As an Ambulatory Care Nurse with Alameda Health System, you will play a vital role in providing high-quality patient care in an outpatient setting. Under the direction of experienced professionals, you will perform a range of nursing duties, including monitoring, treating, and educating patients and their families about various medical conditions. Your expertise will be essential in recording medical histories and symptoms, performing diagnostic tests, and analyzing results. You will also operate medical equipment, administer treatment and medications, and make assessments to create a plan of care. Additionally, you will provide lead direction to other nursing and paraprofessional personnel and perform other duties as assigned and required by the assigned clinical setting.

Key Responsibilities
  • Organize and supervise the quality of care provided to patients and families by co-workers for a given work period, taking responsibility as a team leader when assigned.
  • Participate in the implementation of patient care standards, infection control standards, and quality assurance criteria, assisting with conducting studies and participating in unit and other meetings.
  • Provide patient-centered care that is respectful of and responsive to individual patient preferences, needs, and values, delivering excellent customer service for both internal and external customers.
  • Collaborate with patients and other healthcare providers to deliver coordinated, high-quality care.
  • Perform nursing care following prescribed roles, policies, protocols, and procedures.
  • Prepare, educate, and monitor patients undergoing procedural sedation.
  • Critically evaluate and use good judgment in unusual or emergent situations.
  • Complete patient assessments and triage, record patient histories, symptoms, and observations.
  • Clinical skills include vital signs, procedural sedation management, cleaning and use of medical equipment, Point of Care Testing, and phlebotomy.
  • Provide clinical support and direct case management, including patient education, goal setting, self-management teaching, and coaching.
  • Effectively utilize population health methodologies, such as patient registry, in-reach, out-reach, care protocols, and team-based care, to improve patient and population outcomes.
  • Provide care coordination of patient-centered care, based on evidence-based guidelines, coordinating care based on patients' healthcare needs and treatment recommendations to establish and maintain a plan of care jointly created and managed by patient, family, and care team.
  • Administer medications, immunizations, perform medication reconciliation, titrate medications, and manage refills per clinical guidelines.
  • Perform accurate assessments of patient conditions (face-to-face or through telehealth) and utilize guidelines or standardized procedures to create an effective plan of care.
  • Participate in Performance Improvement/Quality Control activities.
  • Empathize with vulnerable populations and underserved patients.
Requirements

Any combination of education and experience that would likely provide the required knowledge, skills, and abilities, as well as possession of any required licenses or certifications, is qualifying.

Graduate of an accredited Registered Nursing Program.

Preferred: Bachelor's degree in Nursing or higher.

Six months experience as a Clinical Nurse I with AHS or the equivalent of one year full-time recent experience at a comparable level in an acute care hospital or in the area of specialty; (if experience is not recent, completion of an approved refresher course may be acceptable).

Valid license to practice as a Registered Nurse in the State of California.

Advanced Cardiac Life Support Certification issued by the American Heart Association; Completion of moderate sedation competency.

Provider existing (or obtain new, if needed) NPI number.

Preferred: Moderate Sedation Certification.
