Key Responsibilities:

Service Coordination & Support

  • Guarantee the provision of quality assistance for adults with disabilities and brain injuries, focusing on their safety, well-being, and independence.
  • Maintain consistent communication with supervisors and relevant parties.
  • Engage in weekly discussions with nursing staff regarding program operations and client needs.
  • Facilitate the development and implementation of personalized service plans, monitoring progress towards established goals.
  • Organize meaningful activities that promote social engagement and skill development.
  • Collaborate with nursing staff on medical service delivery.
  • Ensure nutritional requirements are met according to individual preferences and dietary guidelines.
  • Encourage healthy living practices among individuals served.
  • Adhere to medication administration policies and maintain accurate documentation.
  • Assist individuals with personal care tasks as necessary.
  • Support individuals in completing daily living activities, including cooking and cleaning.
  • Ensure adequate staffing coverage in accordance with funding standards.

Positive Behavior Support Implementation

  • Apply Positive Behavior Support practices to foster a supportive environment tailored to individual needs.
  • Participate in Universal PBS Meetings regularly.
  • Provide training and feedback to staff on PBS practices.
  • Empower individuals to make choices in various settings.

Community Engagement

  • Promote healthy relationships and community integration through individualized activities.
  • Assist individuals in planning community-based events.
  • Facilitate opportunities for individuals to build friendships and connections.
  • Create inclusive social environments that foster belonging.
  • Guide staff in enhancing their community engagement skills.

Staff Development & Management

  • Serve as an advocate for individuals and staff.
  • Participate in the hiring process and ensure timely documentation.
  • Utilize electronic systems for HR functions.
  • Provide ongoing training and supervision to staff.
  • Monitor compliance with training requirements.
  • Conduct staff evaluations and ensure adherence to policies.
  • Facilitate staff meetings and training sessions.

Quality Assurance

  • Identify and address program needs promptly.

Qualifications:

  • Bachelor's degree preferred or three years of relevant experience.
  • Valid driver's license required.
  • Two to three years of supervisory experience preferred.
  • Basic computer skills necessary.
  • Strong critical thinking and communication abilities.
