We are seeking an experienced Hospice Registered Nurse to join our team at Liberty Hospice. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with terminal illnesses and their families.
Key Responsibilities:- Apply nursing theories and concepts to deliver evidence-based care
- Coordinate physical care of patients with families, physicians, and the hospice team
- Communicate effectively with patients, families, and agency personnel
- Participate in IDT meetings and quality reviews as scheduled
- Current RN licensure in the state of practice
- Successful completion of a nursing education from an approved school of nursing
- Two years of clinical nursing experience and at least one year of hospice nursing experience preferred
- Knowledge of home health and hospice care programs and the nurse's role as a member of the patient care team
- 12 hours of continuing education annually
- CPR certification
- Ability to accept travel assignments and work weekends, evenings, and holidays
