A Home Health Aide plays a vital role in delivering personalized care to patients in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of others.

Key Responsibilities:
  • Provide direct patient care under the supervision of a Registered Nurse, adhering to the Aide Plan of Care (POC).
  • Assist patients with self-administered medications, ensuring accurate dosing and observation.
  • Take accurate temperature, pulse, and blood pressure measurements, recognizing and reporting abnormal results to the supervisor.
  • Support patients with daily living activities, including bathing, grooming, skin care, and oral care.
  • Assist with feeding patients, promoting basic nutrition principles and observing food and fluid intake.
  • Help patients with toileting, positioning, and transferring, ensuring their safety and comfort.
  • Document interactions, care rendered, and changes in patient condition, maintaining accurate records.
  • Practice infection control principles, maintaining a clean and safe environment.
Requirements:
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation.
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course.
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
