Job Summary

Optum, a part of the UnitedHealth Group family of businesses, is seeking a Nurse Practitioner or Physician Assistant to join our Home-Based Medical Care team in Hartford and Litchfield County, CT. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

Responsibilities
  • Conduct comprehensive assessments of patients and develop individualized care plans
  • Provide ongoing care and management of patients' conditions, including medication management and education
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and experience
Requirements
  • Current state RN/NP or PA license
  • Completion of Nurse Practitioner or Physician Assistant accredited program
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
Preferred Qualifications
  • 2+ years of clinical experience, primary care, home-based medical care, geriatrics or palliative care of emergency/urgent care
  • Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems
