Responsibilities- Coordinate Patient Care: Collaborate with the healthcare team to develop and implement individualized patient care plans, ensuring seamless transitions and optimal health outcomes.
- Assess and Prioritize Patient Needs: Conduct thorough assessments, identify patient priorities, and make informed decisions to address their physical, emotional, and social needs.
- Communicate Effectively: Engage with patients, families, and healthcare professionals to ensure clear communication, address concerns, and promote a positive patient experience.
- Administer Medications and Treatments: Safely administer medications, perform venipuncture, and initiate IV lines as needed, adhering to hospital policies and procedures.
- Monitor Patient Status: Continuously monitor patients' clinical status, recognizing and responding to changes, and escalating concerns as necessary.
- Active RN License: Possess an active Registered Nurse license in the state of New York.
- Education and Certifications: Hold a Bachelor's Degree in Nursing and possess current certifications in BLS, ACLS, and PALS.
- Experience: Minimum 2 years of experience in a medical-surgical setting, with at least 1 year of experience in coronary care practice.
