Job Summary

We are seeking a highly skilled Behavioral Health Therapist to join our team at Mid Michigan Health. As a key member of our primary care treatment team, you will play a vital role in identifying, assessing, and managing patients with medical and behavioral health problems.

Responsibilities
  • Assessment and Triage: Assess the clinical status of patients referred by primary care providers, detect at-risk patients, and intervene to prevent further psychological or physical deterioration.
  • Clinical Intervention: Assist primary care providers in recognizing and treating mental disorders and psychological problems, consult with physicians as necessary, and refer cases to other behavioral health providers for longer-term care.
  • Patient Engagement: Facilitate and ensure patient engagement and follow-up care, work with primary care teams to treat and manage patients with chronic emotional and/or physical health problems efficiently and effectively.
  • Documentation and Reporting: Systematically document treatment response, monitor patients for changes in clinical symptoms and treatment side effects or complications, and improve clinical outcomes with high-prevalence medical and mental health conditions.
Program Development and Education
  • Program Development: Assist in program development, obtain and keep available patient education materials about common mental health and substance use disorders, and provide and update local treatment options and education/resources for patients and their families.
  • Patient Education: Provide patient education and training on integrated care models, care team information, and relevant theories of family dynamics, crisis intervention, strengths-based, solution-focused, and meaning of illness.
Requirements
  • Education: Master's degree in social work or a related field from an accredited college or university.
  • Licensure: Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan, or equivalent experience and training.
  • Skills: Knowledge of psychology or social work, casework principles, diagnosis, assessment, crisis intervention, treatment, and transition planning, as well as interpersonal skills and ability to communicate effectively with individuals and groups.
Working Conditions

Exposure to stressful situations, including those involving public contact, trauma, grief, and death. Ability to wear personal protective equipment, move freely about the facility, and perform the functions of the job as outlined in the job description.
