We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Job Summary:

The successful candidate will be responsible for providing comprehensive skilled services as ordered by the attending physician. This will include observing, recording, and reporting patient reactions, changes, and progress to the attending physician and any appropriate agency staff. Additionally, the Physical Therapist will be responsible for performing OASIS assessments to develop an individualized plan of care and making adjustments as needs change.

Key Responsibilities:
  • Provide skilled physical therapy as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Perform OASIS assessments to develop an individualized plan of care and make adjustments as needs change.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
  • Help decrease re-hospitalizations by front-loading visits for high-risk patients and teaching the patient on preventative measures and making good decisions.
  • Counsel patients and families on the disease/injury and how to manage in all phases of physical therapy.
  • Instruct patients and families on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Oversee and supervise therapy services of patients provided by PTAs and when appropriate, nurse assistants.
  • Communicate with PTAs and nurse aides on supervisory visit results.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
Requirements:
  • Physical Therapist with a current license in the state of employment.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
