Job Title: Home Health Physical Therapist

LHC Group is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Kettering, Ohio.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist to provide high-quality patient care in a home health setting. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing ongoing care and support to patients in their homes.

Responsibilities:
  • Evaluate patient functional status and physical therapy needs, and assist the physician in developing the therapy plan of care.
  • Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in the state of Ohio.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
