Optum Home & Community Care is creating a new model of care that addresses the physical, mental, and social needs of patients. As a Senior Community Care Nurse Practitioner, you will be part of an interdisciplinary team providing care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical needs
  • Manage patients through acute medical situations or exacerbations of chronic conditions
  • Ensure accurate and complete documentation of patient information
  • Communicate and collaborate with the interdisciplinary care team
  • Provide patients and caregivers with counseling and education

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state of New York
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
  • Current, active DEA licensure/prescriptive authority

Preferred Qualifications:

  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
