LPN - Clinic I (Meds) - Outpatient Care Specialist

As a Licensed Practical Nurse (LPN) in our Clinic I (Meds) team, you will provide direct and indirect nursing care to patients in an outpatient setting. Under the direction of a Registered Nurse and medical orders from a Practitioner, you will be responsible for orienting and serving as a resource to LPN Clinic/Meds I as needed.
Key Responsibilities- Review and prepare medical records for assigned patients, initiate follow-up as requested, and review patient needs to identify and prepare orders for authorization by the Practitioner.
- Observe, collect, and document new information on patients, inform RN or Practitioners regarding incomplete data, new observations, or abnormal/unusual data.
- Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by the Practitioner, and prepare medications for the Practitioner to use.
- Provide laboratory, x-ray, and other reports to the Practitioner for review, notify patients of normal results as directed by the Practitioner, and consider special needs of different age groups when providing care.
- Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards.
- Collaborate with RN/Practitioner to revise care plan when needed, demonstrate and apply competency in basic nursing skills and use of equipment, and maintain current knowledge and skills needed to perform position.
- Participate in orientation of new personnel, orient and serve as a resource to LPNs and unlicensed personnel as needed, and demonstrate sufficient knowledge, skills, and abilities in area of specialization.
- Perform other duties and accept responsibility as assigned within scope.
- Twelve (12) months (2080 hours) LPN experience.
- Hawaii State Board of Nursing approved curriculum for LPN licensure.
- Licensed Practical Nurse (Hawaii) required at hire.
- Basic Life Support from American Heart Association.
- Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
- Acute care nursing experience.
- Demonstrated knowledge of and skill in word processing and spreadsheet PC applications.
