**Key Responsibilities**

* Conduct comprehensive assessments and provide skilled nursing care to patients in their homes, ensuring care plans are executed with precision.
* Complete OASIS documentation accurately to ensure proper patient care planning and compliance with regulatory standards.
* Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team.
* Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care.
* Provide weekend on-call support on a rotational basis, addressing urgent patient needs.
* Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health.

**Requirements**

* Active Connecticut Registered Nurse (RN) License.
* Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
* Minimum of 1 year of experience as an RN in a home health setting.
* Proficiency in OASIS and Homecare Homebase (HCHB) software required.
* Reliable transportation and a valid drivers license.
* Ability to complete Joint Commission onboarding standards.
