Job Summary:

We are seeking a skilled and compassionate Registered Nurse to provide high-quality care to adult and geriatric patients in the comfort of their homes. As a valued member of our team, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.

Responsibilities:

  • Conduct comprehensive assessments and provide skilled nursing care to patients in their homes.
  • Complete OASIS documentation accurately to ensure proper patient care planning and compliance with regulatory standards.
  • Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team.
  • Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care.
  • Provide weekend on-call support on a rotational basis, addressing urgent patient needs.
  • Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health.

Requirements:

  • Active Connecticut Registered Nurse (RN) License.
  • Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
  • Minimum of 1 year of experience as an RN in a home health setting.
  • Proficiency in OASIS and Homecare Homebase (HCHB) software required.
  • Reliable transportation and a valid driver's license.
  • Ability to complete Joint Commission onboarding standards.
