Job Title: Hospice RN On-Call

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as an On-Call RN. As a member of our team, you will provide high-quality patient care and support to patients and families in their homes, hospitals, and other healthcare settings.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans to meet their physical, emotional, and spiritual needs.
  • Provide skilled nursing care and interventions to patients and families, including wound care, medication management, and symptom management.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
  • Document patient care and progress in a timely and accurate manner, adhering to regulatory and organizational standards.
  • Participate in ongoing education and training to enhance clinical knowledge and skills, staying up-to-date on best practices and evidence-based care.
  • Contribute to a positive and supportive work environment, promoting a culture of respect, empathy, and compassion.

Requirements:

  • Current licensure as a Registered Nurse in the state of Florida.
  • Minimum two years of experience as a Registered Nurse, preferably in a hospice or palliative care setting.
  • Strong assessment and communication skills, with the ability to work effectively in a fast-paced environment.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.

Work Environment:

This is a full-time, on-call position, requiring flexibility and availability to work varied shifts, including evenings, weekends, and holidays. The successful candidate will be required to travel to patient homes, hospitals, and other healthcare settings, using their own vehicle and maintaining a valid driver's license.
