Hospice Nurse On-Call

We are seeking a skilled Registered Nurse to join our team as an On-Call RN for our Hospice Care program. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and families in need.
Responsibilities- Assess patient needs and develop individualized care plans to maximize comfort and health.
- Communicate effectively with attending physicians, other healthcare professionals, and patients/families to coordinate care and services.
- Maintain accurate and up-to-date patient records, including problems, plans, actions, and goals.
- Provide education and support to primary caregivers, volunteers, and employed caregivers to ensure optimal patient care.
- Participate in regular team meetings to review patient cases, share knowledge, and provide feedback.
- Supervise and document hospice aide delivery of care every 14 days.
- Inform the Administrator of unusual or potentially problematic patient/family issues.
- Assist other nurses as needed and provide 24-hour, seven-day-a-week coverage to patients/families.
- Provide support at time of death and perform bereavement assessment if requested.
- Participate in agency and community programs to promote professional growth and understanding of hospice care.
- Current license to practice as a registered professional nurse in the state of Florida.
- Minimum two years of recent varied work experience as a professional nurse, preferably one year in an acute setting.
- Demonstrated ability to assess and respond to patient/family needs in varied settings.
- Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
