Bilingual Case Manager

Medix is seeking a highly motivated and compassionate Bilingual Case Manager to join our team. As a Case Manager, you will work closely with clients to develop and implement individualized service plans that address their housing goals, income, and personal needs.
Key Responsibilities- Conduct intake and needs assessments for clients, collaborating with them to develop and implement individualized service plans.
- Provide referrals, linkages, and support to resources that help clients achieve their goals, including medical, mental health, and vocational services.
- Maintain accurate and up-to-date client files, adhering to program and PATH policies.
- Develop therapeutic relationships with clients and systems to enhance engagement and retention.
- Provide crisis intervention and risk assessment services, utilizing motivational interviewing and trauma-informed care.
- Maintain current knowledge of community resources and provide comprehensive, wrap-around services to clients.
- Demonstrate a commitment to diversity, equity, and inclusion through inclusive service delivery and continued learning.
- Bachelor's degree in Social Work, Psychology, or related field.
- 2+ years of experience in case management or a related field.
- Bilingual proficiency in English and Spanish.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
