Salary: $120,000 - $180,000 per year

About the Role:

The successful candidate will be responsible for assessing patients' strengths and weaknesses to create effective treatment plans. You will work closely with an interdisciplinary team to provide personalized care and support patients in achieving their goals. Key Responsibilities:

  • Collect and record patient care data accurately and efficiently

Requirements:
