Salary: $120,000 - $180,000 per year
About the Role:The successful candidate will be responsible for assessing patients' strengths and weaknesses to create effective treatment plans. You will work closely with an interdisciplinary team to provide personalized care and support patients in achieving their goals. Key Responsibilities:
- Collect and record patient care data accurately and efficiently
- Develop and implement comprehensive treatment plans based on evaluation results and best practices
- Collaborate with patients and their support system to ensure seamless care coordination
- Provide education and training to patients and their families on therapeutic exercises and self-care techniques
- Maintain accurate records of patient progress and outcomes
