About the Role

The Physical Therapy Assistant plays a vital role in the interdisciplinary team at CareOne, responsible for managing the care of residents and ensuring quality outcomes. This position requires a strong commitment to delivering exceptional patient care and collaborating with other healthcare professionals to achieve optimal results.

Key Responsibilities
  1. Resident Care Management: Provide and direct resident rehabilitation care, adhering to all relevant standards and regulations.
  2. Treatment Planning: Evaluate resident needs and establish effective treatment plans, ensuring efficient and cost-effective care.
  3. Team Collaboration: Manage and coordinate the supervision of treatment plans with all staff and other disciplines as indicated.
  4. Screening and Feedback: Participate in resident screening and provide feedback to designated supervisors on clinical and rehabilitation issues.
  5. Center Processes: Participate in all center-directed processes and services, demonstrating effective management of treatment and non-treatment related activities.
  6. Clinical Documentation: Perform necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
  7. Billing and Marketing: Maintain accurate and timely billing records and assist with marketing efforts as directed.
Requirements
  • Education: Bachelor's Degree in Physical Therapy
  • Licensure: Current and Active Physical Therapy Assistant license within the state of operations
  • Experience: Prefer experience/knowledge in older adult rehabilitation and knowledge of governmental and regulatory guidelines in designated healthcare settings
