Job Title: FBR Family Support Specialist

Job Summary:

The FBR Family Support Specialist will work with families to provide case management, home visits, and substance screens. The ideal candidate will have a strong understanding of child development and trauma, as well as excellent communication and organizational skills.

Key Responsibilities:

  • Collaborate with clinicians to provide case management and develop treatment plans
  • Assist with the implementation of treatment plans and modify as necessary
  • Provide discharge planning and connect families with community services and programs
  • Complete documentation and reports required by agency and program policies and funding sources
  • Develop and maintain collaborative relationships with community agencies and organizations

Requirements:

  • BA in social work, sociology, psychology, or comparable human service program
  • Two years' experience in a comparable position in a human services agency preferred
  • Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel
  • Experience providing in-home services, case management, coordinating systems of care, and working with complex family systems strongly preferred
