Job Summary

We are seeking a highly skilled and detail-oriented MDS Coordinator to join our team at Knife River Care Center. As a key member of our interdisciplinary staff, you will play a critical role in ensuring that our residents receive the highest level of care and achieve their full potential.

Key Responsibilities
  • Coordinate with interdisciplinary staff to develop and implement care plans that meet the unique needs of each resident.
  • Ensure that all MDS, care plans, and CAA's are completed accurately and on time, in accordance with federal, state, and local regulations.
  • Review resident charts for completeness and accuracy, and make recommendations for improvement.
  • Collaborate with the interdisciplinary team to develop and implement a monthly MDS schedule, and distribute it to team members and the Director of Nursing.
  • Oversee the completion of all sections of the MDS, CAA's, and care plans, and ensure timely transmission to CMS and the state.
  • Assist with the coordination of care plans for residents within your caseload, including regular reviews with involved department heads.
  • Alert nurse managers and charge nurses to any needs or problems a resident may have, and assist in the follow-through and implementation of needed changes.
  • Participate in QAPI teams and all nursing department meetings, and assist with the development of resident care policies and procedures.
Requirements
  • Currently licensed as an RN or LPN in the state of North Dakota.
  • Must be IV certified or working towards IV certification, and hold current CPR certification.
