Description

Coordinates the interdisciplinary staff to ensure that residents achieve the highest practicable level of well-being. Ensures the completeness of MDS, care plans and CAA's in accordance with Federal, State, and Local standards, guidelines and regulations. Reviews assigned resident charts for proper documentation and completeness.
  • Shall function as MDS/Medicare case coordinator when on duty.
  • Collaborate with Interdisciplinary Team to formulate a monthly MDS schedule, and distribute to team members and DON.
  • Oversight of all sections for MDS, CAAs, care plan update, as well as timeliness for MDS transmission.
  • Responsible for/or assists with regular MDS transmission to CMS and State; then follow up to verify acceptance report. Make necessary adjustments for MDS not accepted by State or CMS.
  • Will be responsible for follow up on any appeals related to rate changes/classification changes.
  • Review resident charts for adequate documentation as part of preparation for Care Conference.
  • Work closely and in conjunction with the DON, Business Office Manager and Interdisciplinary Team.
  • Attends nurse manager meetings, and weekly Medicare and Event meetings for improved communication with MDS/Medicare potential needs.
  • Assists with coordination of care plans for residents within caseload, including heading regular review with involved department heads. Residents will be visited regarding care plan review and family members will be invited per resident request.
  • Alert Nurse Manager(s) and charge nurses of any needs or problems a resident may have that has been noted or discussed during care conferences, assist in the follow through/implementation of needed changes to address/correct problems or issues discussed.
  • Responsible to assist in education of floor staff on any problems noted with documentation accuracy or understanding.
  • Will participate in Medicare meetings to include Medicare A and B, and restorative nursing which may result in modification of MDS schedule.
  • May be subject to assist with covering floor hours as Charge Nurse at the discretion of the Director of Nursing.
  • Participate in QAPI teams/team leader and all nursing department meetings.
  • Shall assist and have input regarding Resident Care Policy and Procedures.
  • Complies with the Job Duties and Responsibilities of the Charge Nurse Job Description when working as the Charge Nurse.
  • Will participate in other Nurse Manager duties such as weekend/holiday call, doctor/consult rounds, resident admits/discharge planning/education, restorative nursing/therapy participation.


Requirements

Currently licensed as RN or LPN in the state of North Dakota. Must be IV certified or working towards IV certification and hold current CPR certification.
