Position Overview:The Health Community Advocate (HCA) serves as a vital resource for patients and the community, facilitating access to essential services such as health insurance, nutrition, housing, quality healthcare, and health-related information. The HCA collaborates closely with healthcare providers, primary care teams, and various organizations to enhance patient care and health outcomes. Additionally, the HCA functions as an Outreach & Enrollment, Certified Application Counselor (CAC) for the Affordable Care Act, assisting patients and non-patient residents in navigating eligibility and enrollment processes for the Marketplace and sliding fee program.

Key Responsibilities:Patient Support:
  • Assist health center patients with unique needs in applying for housing assistance, Medicaid, Medicare, and other necessary forms.
  • Act as a liaison among clients, healthcare providers, community organizations, hospitals, government entities, and insurance brokers, playing an integral role in care coordination and case management.
  • Guide patients/clients in utilizing available resources, including appointment scheduling and application completion for eligible programs.
  • Identify resources to help clients overcome barriers to care, such as transportation, housing, and childcare.
  • Facilitate communication and coordinate services among healthcare providers.
  • Continuously enhance knowledge of community resources, services, and programs available.
  • Maintain strict confidentiality and adhere to all HIPAA regulations.

Outreach & Enrollment:
  • Assist patients and non-patients in enrolling in the Health Insurance Marketplace during open enrollment periods or when qualifying life events occur.
  • Help clients determine eligibility based on qualifying life events.
  • Upload income verification or necessary documentation to the Marketplace application.
  • Assist clients in making updates to their Marketplace applications due to changes in income, family size, employment, or address.
  • Organize outreach events to educate the community about special enrollment opportunities, open enrollment periods, and insurance literacy.
  • Promote enrollment events through local media channels.
  • Monitor and report on assistance provided, submissions, and enrollment statistics to Eastport Health Care, Inc. on a monthly basis.

Minimum Qualifications:
  • Associate's Degree or equivalent experience.
  • Valid driver's license, clean driving record, and personal vehicle insurance.
  • Experience in presentations and public speaking.
  • Ability to collaborate effectively with colleagues, patients, and the public.
  • Proficient in using office equipment such as scanners, laptops, fax machines, and copiers.
  • Skilled in Windows, Outlook, Microsoft Office, and PDF file management.
