Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing dialysis care to patients in an outpatient setting. As an LPN, you will play a vital role in delivering safe and effective dialysis therapy, while also providing emotional support and education to patients and their families.

Responsibilities
  • Provide direct patient care, including administering and monitoring dialysis treatments, as prescribed by the physician.
  • Conduct pre, interim, and post-treatment reviews of patient condition, and document any changes or concerns.
  • Administer intravenous medication, as ordered by the physician, and ensure proper disposal of medications.
  • Participate in the implementation and evaluation of patient care plans, in collaboration with the healthcare team.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
  • Maintain accurate and detailed records of patient care, including treatment plans, progress notes, and medication administration.
  • Collaborate with the healthcare team to identify and address any patient concerns or issues.
Requirements
  • Graduate of an accredited School of Practical Nursing (LPN, LVN)
  • Current appropriate State licensure
  • One year medical-surgical nursing experience preferred
  • Hemodialysis experience preferred but not required
  • ICU experience preferred but not required
  • Successfully complete a training course in the theory and practice of hemodialysis
  • Successfully complete CPR Certification
  • Ability to provide coverage at area facilities during times of short-staffing or as required
